What a useful SMS contains
Policies, responsibilities, procedures, reporting arrangements, emergency preparedness, maintenance controls, documentation, internal verification and management review should work together rather than exist as disconnected documents.
The role of shore management
Effective implementation requires resources, clear authority, communication with ships and timely support when personnel identify hazards or operational barriers.
Learning from nonconformities
Repeated findings should trigger root-cause analysis and system-level corrective action. Closing a form is not the same as closing the problem.
